Prior to starting work on a project, you'll need to complete the following forms and send them to jennifer@cypresssearch.com or fax 678.623.0054:

• State of Georgia Employee's Withholding Certificate (G-4)
• U.S Department of Justice INS Verification (I-9)
• Employee's Withholding Allowance Certificate (W-4)
• Direct Deposit Form

Once you begin a project, you'll need to fill out and submit a timesheet each week.
